AVIVA Contracting Paperwork Has Been Updated The AVIVA agent contracting paperwork has been recently updated. You can find the AVIVA contracting paperwork on the agent website in the Infocenter under "Agent Contracts and Licensing Forms." Included with the contract is a checklist to help ensure you have all the required forms for submitting the contract. Once you have filled all the necessary fields on the contracting forms and made sure all forms are included, please be sure to fax it to the Licensing Department at 1-888-256-8134. Please know, any contracts submitted that are not the updated contract will be returned to the office and updated contracting paperwork will be requested.
