10-19-09 Completing Paper Scope of Appointment Forms To meet the CMS requirement to complete a Scope of Appointment for each Medicare Advantage or Prescription Drug Plan appointment, you have the option of either completing a paper Scope of Appointment Form or recording the information through our new telephone recording service, ExpressLine. We recommend - but do not require - that agents opting for paper forms use one of our Universal American product-branded Medicare Advantage Scope of Appointment forms. These are included with supply orders and can also be downloaded from AgentLink. Please use the form associated with the product you will be presenting: PFFS, PPO, HMO or PDP. - Our MA Scope forms have spaces to include the time and date of the appointment, even though CMS does not require this information. This space should be used for noting the time and date of the pending appointment, NOT the time and date when the appointment was scheduled.
- The PDP Scope forms do not have the time and date because this information is not required.
- In addition to our own Scope of Appointment forms, Universal American will also accept a properly completed Scope form from another carrier, provided it has all of the required elements that CMS requires and are included on the UAM forms.
- Universal American will also accept a generic Scope form, again provided it has all of the required elements.
Using our Scope of Appointment recording service through ExpressLine is a simple and effective way to meet this requirement, and is the preferred method.
