Secure Guard II Replacement Forms The new Pennsylvania Life Secure Guard II applications use the NAIC Model replacement question wording in most states. Because of the change, we have had to change or add a replacement form to your required forms list to stay in line with the new model. Please review the Secure Guard II Replacement Form spreadsheet to find out if a form has been added or replaced in your state. Please know that the form numbers in red are indication that you may no longer use that form. The new forms are available for download on the agent website.
