Your Health Risk Assessment is Key to Good Health and Discounts All employees participating in our medical plans are encouraged to complete an annual personal health assessment.  Plan participants who selected the Wellness or Non-smoker rates for 2010 MUST complete a new Personal Health Risk Assessment before December 31, 2009 to receive that rate. Failure to complete your Health Risk Assessment will increase your contribution rate.   Please visit www.myuhc.com to complete your Personal Health Risk Assessment.  If you are newly eligible beginning January 1, 2010, please follow the prompts to register. Once registered, you will have the opportunity to complete the online assessment.
