Dear Juan Curbelo:
We have completed our processing of your termination, effective December 23, 2009, per the request of BONITA SPRINGS, MASCAL CAR3. Your contract as well as your authority to represent the Company(s) listed below, subject to the applicable notification period, has been terminated.
A notification has been submitted to the applicable state regulatory agency informing them accordingly of this action.
If you meet the vesting of commission requirements, as it applies per the terms of your agent contract(s), commission payable to you under any UAC subsidiary company contract and/or affiliate company, less any outstanding debit balance, will be paid on an earned basis only, one time per month. If commissions do not exceed the minimum check amount, then commissions are accumulated until a month end where commissions exceed the minimum check amount.
Please review the following bullets and proceed accordingly:
- Please promptly return to the Company, all policies, policyholder lists, sales aids, applications, supplies and any other item in your possession that are exclusive property of the Company.
- If not vested, you will be held accountable for any debit balances incurred by you or any representative contracted by you. If applicable, we will initiate our formal debt recovery process, which includes reporting documented debit balances to Vector.
- If vested and future commissions cannot satisfy your current debit balance in an acceptable time frame, we may initiate a formal debt recovery process and request you to repay any documented debit balance immediately. Such notice will come under separate cover.
We regret this action but wish to extend our thanks for your efforts on behalf of our Company.
Sincerely,
Universal American Agent Licensing Department
